GH Receptor Structure and JAK2/STAT5 Signaling

Human growth hormone binds to the GH receptor (GHR) as a 1:2 ligand:receptor complex, inducing receptor dimerization. This activates the Janus kinase 2 (JAK2), which phosphorylates the intracellular domain of GHR. Phosphorylated GHR recruits and activates STAT5 (Signal Transducer and Activator of Transcription 5), which dimerizes, translocates to the nucleus, and induces transcription of target genes including IGF-1. The crystal structure of the hGH-GHR complex (PDB: 3HHR) revealed that two distinct receptor-binding sites on hGH (Site 1 and Site 2) mediate sequential receptor engagement.

Pulsatile GH Secretion Patterns

Endogenous GH secretion follows a pulsatile pattern regulated by the interplay of hypothalamic GHRH (stimulatory) and somatostatin (inhibitory). In humans, the largest GH pulse typically occurs during slow-wave sleep. 24-hour GH profiling studies using deconvolution analysis demonstrated that approximately 70% of daily GH output occurs during nocturnal pulses. Pulse amplitude, frequency, and interpulse nadir levels vary with age, sex, and metabolic status.
